For every record-breaking achievement by Premier League champions Manchester City there have been several episodes at the other end of the scale.

Swansea’s end-of-season collapse

Swansea’s survival bid looked to be back on track when boss Carlos Carvalhal took 17 points from his first nine games after replacing Paul Clement just after Christmas. But the Swans went on to pick up just three points from their next nine league matches, losing all of their final five fixtures to drop into the Championship after seven years in the top flight.
