In October 2014, Jason Conroy murdered Melissa Mathieson, 18, at Alexandra House, a Bristol home providing special care for adults.
The Serious Case Review, published by Bristol Safeguarding Adults Board, said failure to act on previous warnings of Conroy’s behaviour had ultimately led to Miss Mathieson’s death.
Conroy was jailed for life in October 2015.
He had been first placed off-island in 2003 in a residential school.
According to the review, Conroy attempted to kill his mother twice; in 2007 tried to strangle a young member of staff while at a residential school, saying he wanted to have sex with her; and in March 2013 sexually assaulted and then tried to strangle a teacher at a residential school.
Health & Social Care president Heidi Soulsby said the review ‘highlights some very distressing and tragic events’ where a ‘highly vulnerable young woman lost her life at the hands of a young man for whom the States was responsible’.
‘I wish to extend my sincere condolences on behalf of the Committee for Health & Social Care to the families and friends of both of the young adults concerned,’ she said.
