Health transformation proposals announced
HEALTH & Social Care has today published its proposals to transform its services.
The 10-year plan aims to provide a greater integration between care providers in the Bailiwick.
Change is needed, Health said, if costs of services in the Bailiwick were not to increase by some £20m.
Its proposals for change ensure that islanders are at the heart of the system, it said, and are based on a 'Partnership of Purpose' among providers, which is focused on prevention and offering greater support in the community.
If agreed by the States, the proposals will also see the introduction of an independent care regulator and a newly established CareWatch, comprising of representatives from the Bailiwick community.
All islanders will also be issued a Care Passport that will set out their entitlement to services.
